Personalised Care and Support Planning (PCSP) is a core component of personalised care.  This one-day, online workshop, will provide participants with the knowledge and skills to put high quality PCSP into practice.   Attendees will receive an in-depth introduction to PCSP, in the context of the NHS Universal Model of Personalised Care, as well as practical advice and skills to implement PCSP into practice. 

Whilst we welcome attendance from any health and social care professional who is involved in writing or reviewing care plans, this course is based on NHS principles, guidelines and practice. 

This course is accredited with the Personalised Care Institute.

Here at WASP, we love supporting health and social care professionals to embed personalised care within their everyday practice.  Personalised care and support planning (PCSP) is one way in which care for people with long term conditions can be coordinated and personalised. 

Working together, care professionals, people with long term conditions and their families/carers, develop care plans that are based on the outcomes that are most important to the person.  To be done well, PCSP requires  a range of skills, including  conversational skills, goals setting, and action planning.  It also requires knowledge about what constitutes a personalised care and support plan, and the steps involved to achieve this.   This one-day workshop will provide participants with the knowledge and skills required for PCSP, alongside an opportunity to reflect on current practice and plan future improvements.  The course is delivered online and involves group work and interactive sessions.   We hope to see you there!

Learning Objectives:

  1. Define Personalised Care and Support Planning (PCSP)
  2. Summarise how PCSP fits with personalised care (universal model and operating model)
  3. Describe the benefits of PCSP and the evidence base that supports it
  4. Demonstrate your understanding of the 6 steps of how you create a personalised care and support plan
  5. State 3 different conversational skills to enable the conversation to develop a personalised care and support plan
  6. Create a reflection of your own practise, including how you will change something to reflect your own learning from the day
